This video presents a case of reoperation for hilar cholangiocarcinoma previously treated by T-tube insertion. A cura-tive resection was performed with common bile duct and caudate lobe resection, hepatic pedicle skeletonization, multiple cholagiojejunostomies and Roux-en-Y biliary reconstruction. The need of accurate tumor staging to avoid overestimation is underlined.
