A patient suffering from malignant obstruction of the hepatic duct confluence infiltrating the common duct was prepared for surgery. During the operative procedure, infiltration of segment I and IV was demonstrated. The final treatment was en bloc resection of the extrahepatic biliary tract with lymphofatty tissue, porta hepatis and segments I and IV. Reconstruction was done by four separate cholangiojejunostomies on one Roux-en-Y jejunal loop.
