Standard 3 is the one that generates the most rework. Relevant financial relationships have to be identified for everyone in control of content, mitigated before that person shapes anything, and disclosed to learners before the education begins. Twenty-four months of lookback. No dollar threshold.

That’s manageable for one activity. Across thirty activities and four departments, it becomes an operational problem — and the failure mode is almost never a missing disclosure. It’s a disclosure that was collected, handled correctly, and can’t be produced on request.

This template gives the process a home: who disclosed what, when, what relationship was identified, how it was mitigated, who reviewed the mitigation, and where the learner-facing disclosure appeared.
