This is a 20 years old female, referred with left thyroid nodule. Ultrasound showed 1.1 cm, vascular, stiff, hypoechoic thyroid nodule and suspicious looking lymph nodes at level II and level IV of the left sided neck. Fine needle aspiration biopsy (FNAB) showed papillary thyroid carcinoma (PTC) with metastatic deposits in the lateral neck lymph nodes. Patient elected to have her surgery done through a hidden incision. All possible risks and complications were thoroughly explained to the patient both verbally and in written form. The procedure went well as planned without complications. Post-operative pathology showed 1.1 cm PTC of the left thyroid lobe with 7/45 positive metastatic lymph nodes in the left lateral neck for PTC. Serum Thyroglobulin levels were 0.4 at 6 months and 0.2 at one year after surgery.
