The purpose of this video is to demonstrate the operative technique and highlight the key technical tips of a retroileal pull-through for the creation of a tension free colorectal anastomosis during a difficult laparoscopic Hartmann's reversal. The patient is a 78 year- old male patient with a history of a lower gastrointestinal bleed from the splenic flexure requiring a Hartmann's procedure with a transverse end colostomy. The video features extensive lysis of adhesions in order to obtain adequate reach in preparation for anastomosis as well as the use of indocyanine green fluorescence imaging to assess colonic vascularity.
