Laparoscopic Conversion of a Banded Plication to a RGBP

This case is that of a 40 yr old white male who had undergone a banded plication 3 years ago. Patient had minimal weight loss with significant dysphagia and reflux. The patient came in emergently for solid food dysphagia. Ct scan revealed that the fundic portion of the plication had slipped. A decision was made to convert the patient to a Lap Roux en Y gastric bypass. Technical details of undoing the plication and conversion to bypass is demonstrated. Patient has done very well with the roux en y gastric bypass and has significant weight loss and resolution of comorbidities.
