51 year old female presented with an acute SBO 2 years after a gastric bypass. She was found to have an impacted cotton ball bezoar in her common channel intra-op, which required small bowel resection. She had stopped taking her vitamins (including iron) and her serum iron level was undetectable, which is why she had pica and was ingesting cotton balls.
