This is a case report of a 64 year old female who had a 20 year history of anterior neck mass, enlarging over the past 5 years. The patient, on neck and chest CT Scan, was found to have thyroid mass with mediastinal extension. The patient underwent Total thyroidectomy with Sternotomy, Excision of Mediastinal Follicular Thyroid Extension under general anesthesia. The patient tolerated the procedure well, was discharged improved with medications.
