A 72 year old lady with a background history of hepatitis B infection was complaining of upper abdomina pain. Contrast CT scan showed an 8cm tumour in thecaudate lobe of the liver compatible with HCC. The platelet count was 95x10^9/L. Pure laparoscopic left lateral resection and caudate lobectomy was performed. Under laparoscopic ultrasound examination, the tumour was identified at caudate lobe abutting on left and middle hepatic veins. Left lateral section of the liver was removed to facilitate a complete resection of caudate tumour. Laparoscopic hepatic parenchymal transection was performed using ultrasonic dissector. The caudate lobe was removed completely with exposure of middle hepatic vein and IVC. Total 5 ports were used and the tumour was retrieved via a Pfannesteil incision. The patient had a smooth recovery and was discharged home on day 5 after the operation.
