Aims Per Oral Endoscopic Myotomy (POEM) has become the treatment of choice for our achalasia patients and with other esophageal motility problems. Using POEM techniques we can readily access the esophageal muscle layers. In this video we would like to support the experience of others, using POEM techniques for Per Oral Endoscopic Tumor (POET) resection of an esophageal leiomyoma Methods A 52-year-old woman has been monitored for several years with an esophageal mass resembling a leiomyoma. This mass was located in the lowest third of the esophagus and during the last year it increased in size making the patient apprehensive. Extensive diagnostic work-up included endoscopies, esophagram, CT scan, endoscopic ultrasound and 3D reconstruction and 3D printing of the mass. She had minor dysphagia, and occasional clinical GERD that responded well with PPIs. Her BMI was 35kg/m2. POET resection was proposed to the patient instead of thoracoscopic approach. Results A POET resection of an esophageal mass was done under general anesthesia. The procedure lasted 54 minutes. The mass was located 35 cm from the incisors and measured approximately 1.6X1.0X0.9 cm. It was done through a 2 cm mucosal incision at 30 cm from the incisors. EBL was minimal. The patient tolerated diet the same day of the procedure, and she was discharged home the day after.
