Chronic groin pain after inguinal hernia repair can significantly impact patients' lives. There are limited surgical options which often require re-operating in a heavily scared surgical field to remove mesh or identify nerves and perform a neurectomy. Here we demonstrate a laparoscopic retroperitoneal triple neurectomy for the management of chronic groin pain. This technique requires a thorough understanding of the retroperitoneal anatomy. The benefits of this approach are that it allows the surgeon to identify the iliohypogastric, ilioinguinal, and genitofemoral nerves outside of the previously operated field and more proximally before there is significant cross innervation.
