A 32 year old white female who presents with a congenital cloaca, not previously repaired, having severe fecal incontinence, recurrent urinary tract infection, and due to the malformation, an inability to conceive. Physical examination confirms the presence a common channel for her rectum and vagina. Without urethral involvement. Workup included an MRI, which confirms normal sacral anatomy. CT abdomen and pelvis confirm left ureteral catheter in place. Manometry demonstrated low resting and squeeze pressures. EMG, and bilateral PNTL were normal. Ultrasonography confirms presence of separated levator muscle with absent anterior perineum. Surgical repair ( video ) demonstrate the planes of dissection for identification of perineal structures, with approximation of perineal and sphincter muscles building up the perineum During post op had superficial perineal infection, that not affect deeper plane repairs. At 3 months follow up she states to have full fecal continence
