Large benign rectal polyps present a treatment challenge. The development of transanal minimally invasive surgery (TAMIS) has expanded the surgical options for resecting these tumors. We present a case of successful TAMIS resection of an 8 cm rectal polyp in a 68-year-old woman that required transanal closure of the peritoneal cavity. Methods: The video presents all the essential operative steps and technical tips for TAMIS resection of a large rectal polyp, with special emphasis on a combination of both endoscopic and open transanal maneuvers to completely resect the polyp en bloc. We also demonstrate the technique for TAMIS closure of a peritoneal defect. Results: All operative details are presented in the video. Relevant anatomy and important endoscopic and open maneuvers are highlighted. Conclusion: Resection of giant rectal polyps can be achieved through appropriate use of the TAMIS platform. A combination of techniques, including transanal closure of the peritoneum, are essential for success.
