An otherwise healthy 29-year old male presented to our hospital with acute appendicitis. At laparoscopy the appendix appeared inflamed and incarcerated within a rare pelvic sidewall intramuscular hernia. After reduction of the hernia contents, the appendix was resected and retrieved. The hernia was within the right pelvic side wall and bordering the ureter and external iliac vessels. The hernia was repaired using bioabsorbable mesh placed in the extraperitoneal space, and secured using fibrin glue. The patient did well in the postoperative period, and remains completely asymptomatic in over a year since his operation. The availability of new bioabsorbable synthetic mesh has allowed for the repair of this rare hernia in a single stage technique, even in the infected field, and with an excellent outcome.
