Gallbladder volvulus is a rare cause of atraumatic, acute abdomen. It occurs predominantly in older people with a female to male sex ratio of 3:1. Torsion of the gallbladder is a rare condition and is very difficult to diagnose preoperatively, despite advances in diagnostic imaging. The patho-physiology involves torsion of the gallbladder around the cystic pedicle. , this diagnosis should be considered in all elderly patients presenting with symptoms suggestive of acute or nonresolving symptoms and signs of cholecystitis, particularly in the absence of gallstones. Treatment is surgical with urgent cholecystectomybecause of the risk of gallbladder perforation. Initial releaseof the torsion on the gallbladder can reduce the risk of dam-age to the biliary tract
